Abstract

This book highlights the work of pioneers in this field, in order to inspire their peers, healthcare leaders and an interested audience. Prior to the respective care practices, the authors describe the core of personalized care and illustrate its connections to value-driven care and evidence-based medicine. The last part of the book addresses organizational and other conditions under which personalized care can thrive. In the Netherlands, substantial work is being done on value-driven care and more is currently being discussed. This book focuses on the benefits of value-driven care for patients and healthcare providers alike. To do so, it explores a range of representative cases and scenarios, such as: Scenario 1: You are able to surgically remove the most difficult tumors and find out one day that you don’t need to do so in all indicated patients. Later you can find out how to recognize when surgery isn’t the best treatment for certain patients. And now your team has a fixed routine to find the optimal choice together with the patient. Scenario 2: You consider yourself to be a patient-centered doctor who is really in touch with their patients – until you ask them to report for themselves on how they are doing. In the "patient-reported outcomes" you see that you have completely missed an important aspect. Later, you can structurally integrate this into your outpatient clinic. Scenario 3: In line with the planning principles of your hospital, you see chronic patients once a year – and of course also 'as needed' – until your data shows you that you see patients too early or too late, but rarely when it would be most helpful. Later you can discover how to offer your consultation exactly on time – based on early signals and often even before the patient calls the clinic. This is what value-driven care is all about: care is of more significance to the patient when it is tailored to his or her unique situation. Recognizing the patient’s needs, preferences and values, and structurally adapting care accordingly – that is the noble profession of value-driven care, which takes us an important step closer to the ideal of personalized care.

Abstract

"In her time, Maria Theresa (1717-1780) was the most powerful woman in the world. She ruled the Habsburg Empire from 1740-1780, an era when empires dominated Europe. She was the sovereign of a vast empire, ruling Austria, Hungary, Croatia, Bohemia, Transylvania, Flanders, and other Habsburg territories, and by marriage she was, among other titles, the Holy Roman Empress. Maria Theresa began her reign at the age of 23 after her father, Emperor Charles VI, died. Immediately after his death, her right to inherit the throne was challenged by most of the sovereign rulers of Europe. Despite setbacks such as the loss of Silesia, her richest province, to her life-long enemy Frederick II of Prussia, Maria Theresa proved to be a highly effective ruler. She initiated financial and educational reforms, promoted commerce, and reorganized the army, all of which strengthened Austria's resources. She was a key figure in the power politics of eighteenth-century Europe and she brought unity to the Habsburg Monarchy and was considered one of its most capable leaders. Maria Theresa and her husband Francis I also had sixteen children, most famously Marie Antoinette. Stollberg-Rilinger's biography challenges many of the myths that surround Maria Theresa's reign, such as that she came to the throne completely naïve and unprepared. Stollberg-Rilinger shows that from early childhood on, Maria Theresa carefully observed what went on in court and how her father acted as a monarch dealing with sovereigns across Europe. She clears away the gendered misconceptions surrounding Maria Theresa's life and, through fresh, critical readings of the source material, reveals the historical reality. She also refutes anachronistic narratives that assume a false continuity between Maria Theresa's time and later periods. Unlike previous biographers, Stollberg-Rilinger is able to paint a detailed portrait of Maria Theresa as Empress, "king," and reformer, and as a mother and master manipulator, by reconstructing the world in which the Austrian Empress lived and reigned"--

Abstract

A captivating historical look at the cultural and artistic significance of shells in early modern EuropeAmong nature's most artful creations, shells have long inspired the curiosity and passion of artisans, artists, collectors, and thinkers. Conchophilia delves into the intimate relationship between shells and people, offering an unprecedented account of the early modern era when the influx of exotic shells to Europe fueled their study and representation as never before. From elaborate nautilus cups and shell-encrusted grottoes to delicate miniatures, this richly illustrated book reveals how the love of shells intersected not only with the rise of natural history and global trade but also with philosophical inquiry, issues of race and gender, and the ascent of art-historical connoisseurship.Shells circulated at the nexus of commerce and intellectual pursuit, suggesting new ways of thinking about relationships between Europe and the rest of the world. The authors focus on northern Europe, where the interest and trade in shells had its greatest impact on the visual arts. They consider how shells were perceived as exotic objects, the role of shells in courtly collections, their place in still-life tableaus, and the connections between their forms and those of the human body. Abstract

This study offers the first complete overview of the remarkable public finances of the Dutch Republic of the United Provinces. Wantje Fritschy has analysed the development and structure of its public revenue and expenditure. She argues that a ‘tax revolution’ and the ‘fiscal resilience’ of the provinces together were more important for its surprising performance than Holland’s public debt alone, and the institutional and economic characteristics of its ‘urban system’ were more important than wealth due to foreign trade. Comparisons with the fiscal systems of three more centralized states - the Venetian Republic, Britain and the Ottoman Empire - underline the crucial importance of long-term ‘urbanization trajectories’ in understanding early-modern fiscal performance. It was not because it was federal that the Dutch Republic collapsed.

Abstract

Het Nederlandse politieke landschap werd van oudsher gedomineerd door drie stromingen: christendemocraten, liberalen en sociaaldemocraten. Lange tijd toonden verkiezingsuitslagen een hoge mate van continuïteit. Daar kwam aan het eind van de jaren zestig van de 20e eeuw verandering in. Vanaf 1994 zette deze ontwikkeling door en sindsdien laten verkiezingen steeds grote verschuivingen zien. Met de opkomst van populistische stromingen, vanaf 2002, is bovendien het politieke landschap ingrijpend veranderd. De snelle veranderingen in het partijenspectrum zorgen voor een overbelichting van de verschillen tussen ‘toen’ en ‘nu’. Bij oppervlakkige beschouwing staat de instabiliteit van vandaag tegenover de gestolde onbeweeglijkheid van het verleden. Een dergelijk beeld is echter een versimpeling, want ook in vroeger jaren konden de politieke spanningen hoog oplopen en kwamen kabinetten regelmatig voortijdig ten val. Er is niet alleen sprake van discontinuïteit, maar ook wel degelijk van een doorgaande lijn.'Een versplinterd landschap' belicht deze historische lijn voor alle politieke partijen die in 2017 in de Tweede Kamer zijn gekozen. De oudste daarvan bestaat al bijna honderd jaar (SGP), de jongste partijen (DENK en FvD) zijn nagelnieuw. Vrijwel alle bijdragen zijn geschreven door vertegenwoordigers van de wetenschappelijke bureaus van de partijen, waardoor een unieke invalshoek is ontstaan: wetenschappelijke distantie gecombineerd met een beschouwing van ‘binnenuit’.
